The Office of Sponsored Programs (OSP) Proposal Routing Form has been recently revised as of May 2006.  We ask that going forward you use the new form for all Grant proposals and IRB submissions.

 

The PI is required to complete the OSP Routing Form (link below) for each proposal submission.  The OSP routing form should be routed for appropriate signatures with the following items attached:

1.      Final OSP reviewed budget

2.      Copy of research proposal (draft copy acceptable)

 

Note: The OSP routing form is required for new submissions, continuation proposals, renewals and transfers. Please note that all proposals submitted through CMH need a routing form, including subcontract proposals in which another institution is the prime grantee and a portion of the work will be subcontracted to CMH.
